Admission Requirements

APPLICATION REQUIREMENTS:

Kinder 1

  • 4 years old on or before June
  • 2 copies of 1 x 1 colored picture
  • child’s Birth Certificate (NSO copy)
  • child’s Baptismal Certificate

Kinder 2

  • 5 years old on or before June
  • 2 copies of 1 x 1 colored picture
  • child’s Birth Certificate (NSO copy)
  • child’s Baptismal Certificate

Grade 1

  • 6 years old on or before June
  • 2 copies of 1 x 1 colored picture
  • child’s Birth Certificate (NSO copy)
  • child’s Baptismal Certificate
  • Original- Pre-elementary Report Card
  • Good Moral Certificate
  • Testing Fee – P 250.00

Grades 2 – 10

  • 2 copies of 1 x 1 colored picture
  • child’s Birth Certificate (NSO copy)
  • child’s Baptismal Certificate
  • Original Report Card
  • Good Moral Certificate
  • Testing Fee – P 250.00

ADMISSION:

All Catholics students are admitted. However, non-Catholics who have strong desire to enroll at Escuela Catolica de San Sebastian would have to sign a written agreement that they must be abide by the school policies regarding a religious instruction and activities.

POLICIES ON STUDENT ADMISSION

New students and transferees are admitted on the basis of the results of entrance examinations, interview, academic records and character information.

  1. Admission in High School

High School students are admitted upon passing the readiness test administered by the Guidance Office

1.1 Submit the following requirements to the Principal’s Office and secure application form

a. Original and photocopy of

Birth Certificate (not hospital certificate)

Baptismal Certificate (must be a Catholic)

Parent’s Marriage Contract

b. Three (3) pcs. 1 x 1 I.D. picture

c. Report Card

d. Certificate of attendance/ enrollment from the previous school

e. Health Certificate

f. Good Moral Certificate

g. Form 137

1.2 Pay the testing fee at the Finance Office

1.3 Proceed to the Guidance Office and get the test schedule

1.4 Come back on the date stated for the interview and written test

1.5 Get the test result at the Principals Office

1.6 Attend Parent’s Orientation as a pre-requisite to enrollment

2.  A former student who desires to re-enroll is treated like any other new student. He takes entrance examinations and passes through the regular procedures.

3.  A transferee/student who seeks admission after enrollment period, (i.e. two(2) weeks after classes have started) must secure permit from DepEd and pass through regular admission procedures.
